SUMMARY

Sinusitis of dental origin represents a unique subset of maxillary sinus infections that arises predominantly from odontogenic sources, posing significant diagnostic and therapeutic challenges within clinical practice. Unlike typical sinusitis, which often results from infectious or allergic etiologies, dental sinusitis stems from infections or pathological conditions affecting the teeth and their supporting structures, particularly the maxillary molars and premolars, leading to inflammation that extends into the adjacent sinus cavity. The clinical presentation of dental-origin sinusitis can be subtle and often overlaps with other sinonasal conditions, making differentiation critical to ensure appropriate management. Common signs and symptoms include facial pain or swelling localized to the maxillary region, dull or throbbing pain that worsens with dental activity, nasal congestion, and purulent nasal discharge, but these may be mistaken for primary sinusitis of non-dental origin. Diagnostic differentiation relies heavily on a combination of thorough patient history, detailed dental examination, and advanced imaging techniques such as panoramic radiographs or cone-beam computed tomography (CBCT), which can reveal dental infections or periapical pathologies contributing to sinusitis. Accurate identification is essential because misdiagnosis can lead to ineffective treatments, prolonged discomfort, and potential complications such as chronic sinusitis or recurrent infections. Effective management of sinusitis of dental origin necessitates a multidisciplinary approach involving dental professionals, otolaryngologists, and radiologists, focusing on both addressing the primary odontogenic source—be it root canal infections, periodontal disease, or dental trauma—and implementing appropriate medical or surgical interventions for sinusitis itself. This paper aims to comprehensively explore the clinical features that distinguish dental sinusitis, elucidate key diagnostic criteria for differentiation from non-dental sinusitis, and review current treatment modalities, emphasizing the importance of integrated care pathways to optimize patient outcomes.

OBJECTIVE

To thoroughly explore the clinical characteristics that distinguish dental sinusitis, clarify the key diagnostic criteria for differentiating it from non-dental sinusitis, and review current treatment modalities, emphasizing the importance of integrated care pathways to optimize patient outcomes.

METHODOLOGY

This review is conducted by systematically analyzing the current literature regarding sinusitis of dental origin, with a particular focus on clinical presentation, diagnostic strategies, differential considerations, and management approaches. Databases including PubMed, Scopus, Web of Science, and Google Scholar are searched for articles published in English between 2000 and 2025 using keywords such as “odontogenic sinusitis”, “sinusitis of dental origin”, “clinical diagnosis”, “differential diagnosis” and “treatment approaches.” Inclusion criteria consist of observational studies, randomized controlled trials, systematic reviews, and meta-analyses that investigate the etiology, diagnostic imaging modalities, microbiological findings, and therapeutic interventions. Exclusion criteria include case reports with limited applicability, studies not focused on odontogenic sinusitis, and articles lacking diagnostic or management relevance. Data are independently extracted by multiple reviewers to ensure consistency and reliability, with special attention given to differentiating odontogenic sinusitis from rhinogenic sinusitis and evaluating the effectiveness of surgical and nonsurgical treatment approaches. The findings are synthesized to provide a comprehensive understanding of best practices in diagnosis and management.

RESULTS

Clinical Features of Sinusitis of Dental Origin:

What are the common signs and symptoms indicating sinusitis of dental origin?

Recognizing sinusitis of dental origin presents a diagnostic challenge due to the significant overlap between its symptoms and those of typical sinus infections, making it difficult to distinguish without targeted dental and radiographic evaluation1. Common signs and symptoms include nasal congestion, pain or pressure in the cheekbones, and headache, all of which are frequently reported in both general sinusitis and odontogenic maxillary sinusitis2. However, dental indicators—such as pain localized to the upper back teeth, particularly when these teeth are associated with extensive restorations, caries, or show sensitivity to percussion—are particularly suggestive, especially when accompanied by radiographic evidence of periapical radiolucency or a widened periodontal ligament space at the apex of the implicated tooth3. The interconnectedness of dental and sinus symptoms is further underscored by cases where dental pain persists with minimal sinus symptoms, or where sinusitis pain radiates to the lower teeth, complicating the clinical picture4. Such complexities emphasize the need for clinicians to maintain a high index of suspicion for dental origins in patients presenting with maxillary sinus symptoms and to utilize both dental and sinus diagnostic modalities. Ultimately, prompt identification and differentiation of the underlying cause are essential for guiding appropriate management and preventing recurrent or chronic sinus disease3.

How does dental sinusitis differ in presentation from other forms of sinusitis?

Despite the considerable overlap in clinical presentation, several distinguishing features set dental sinusitis apart from other forms of sinusitis, primarily due to its odontogenic origin. Dental sinusitis usually manifests as a unilateral process, affecting one side of the maxillary sinus, whereas other types of sinusitis, such as those resulting from viral or allergic etiologies, are more likely to present bilaterally or with diffuse involvement of multiple sinuses5. This unilateral pattern often corresponds to the anatomic proximity of the upper molar teeth to the maxillary sinus, making prior dental procedures—such as extractions, root canals, or implants—a significant risk factor5. Additional hallmark features include the presence of foul-smelling drainage or odors, which are less common in non-odontogenic sinusitis and can guide clinicians to consider a dental source5. Furthermore, the chronic nature of dental sinusitis, with a median symptom duration at presentation of up to six months, contrasts with the more acute course typically seen in other sinusitis types5. This prolonged course is often accompanied by persistent, unilateral symptoms and may include findings such as polyps, severe edema, or purulent drainage with oral flora on culture, further reinforcing the distinction from other sinusitis variants5. The close interplay between dental health and sinus pathology underscores the necessity for clinicians, especially otolaryngologists, to maintain a high index of suspicion for dental sinusitis in patients with unilateral, chronic, or atypical symptoms, and to ensure thorough oral examinations and targeted diagnostic imaging when indicated5. Early recognition and intervention are crucial to prevent complications and guide appropriate multidisciplinary management.

Which diagnostic tools are most effective for identifying dental-related sinusitis?

In the context of diagnosing dental-related sinusitis, the selection of imaging modalities plays a crucial role in both the accuracy of detection and patient safety. Dental panoramic radiographs (DPR) have emerged as a particularly valuable tool, especially when complemented by artificial intelligence (AI) systems that enhance diagnostic precision through advanced object detection algorithms6. Recent advances have demonstrated that AI-assisted analysis of DPR not only bolsters the detection of odontogenic sinusitis but also rivals the diagnostic capabilities of more traditional, higher-radiation modalities such as computed tomography (CT) scans, providing a safer and more cost-effective alternative for routine assessment6. While CT scans remain the gold standard for visualizing the full extent of disease and identifying post-procedural foreign bodies, their use is tempered by concerns regarding radiation exposure, prompting a shift toward modern cone-beam CT (CBCT) systems that significantly reduce dosage and have become routine in dental settings [6]. The integration of image enhancement and deep learning techniques with DPR further bridges the gap between diagnostic efficacy and patient safety by lowering radiation exposure without compromising diagnostic quality6. The interplay between these diagnostic tools underscores the importance of adopting multimodal, AI-supported approaches to accurately identify dental-related sinusitis, while balancing the need for detailed anatomical assessment with minimization of radiation risks. This highlights an urgent need for dental and medical practitioners to prioritize AI-enhanced, low-radiation imaging techniques in routine diagnostics and consider CT or CBCT judiciously in complex cases, ensuring optimal patient outcomes in the management of odontogenic sinusitis.

Differentiation Between Dental and Non-Dental Sinusitis:

What clinical and radiographic criteria help distinguish dental from non-dental sinusitis?

Differentiating dental from non-dental sinusitis requires a multifaceted approach that integrates both clinical and radiographic criteria, with particular emphasis on the anatomical relationship between odontogenic lesions and the maxillary sinus floor. Radiographically, the use of advanced imaging techniques such as sinus Computed Tomography (CT) allows for precise identification and classification of maxillary sinusitis, especially in distinguishing odontogenic maxillary sinusitis (OMS) from its non-dental counterparts7. A key diagnostic criterion is the proximity of dental lesions to the sinus floor; when the distance between the sinus floor and the odontogenic lesion is less than 3.32 mm, there is a significantly higher likelihood that the sinusitis is of dental origin, as this close relationship increases the risk of direct infection spread from the dental source to the sinus7. Furthermore, radiologic features such as complete or partial sinus opacification, irregular air-fluid levels, and mucosal thickening, when localized to the region of the odontogenic lesion, further support a diagnosis of dental sinusitis7. In contrast, the absence of such lesions or a greater distance between the sinus floor and potential odontogenic pathology points toward a non-dental etiology7. The Lund-Mackay (LM) scoring system, which quantifies the severity of sinus opacification, is also a valuable adjunct; higher LM scores correlated with disrupted sinus floor continuity suggest a dental origin, while lower scores and preserved bone structure lean towards non-dental causes7. Given these interconnections, clinicians must meticulously assess both the clinical presentation and high-resolution imaging findings—especially the proximity and continuity of the maxillary sinus floor relative to dental lesions—to accurately differentiate between dental and non-dental sinusitis, ensuring appropriate and targeted management for affected patients.

What are the potential consequences of misdiagnosing dental versus non-dental sinusitis?

The failure to accurately distinguish between dental and non-dental sinusitis carries significant clinical ramifications, particularly due to the risk of untreated dental infections persisting when a dental etiology is overlooked. When dental sinusitis is misdiagnosed as non-dental sinusitis, the underlying dental infection may not be properly managed, which not only perpetuates the patient’s symptoms but also creates a pathway for bacteria to disseminate beyond the oral cavity, potentially leading to systemic infections8. This oversight can have cascading effects, as an unresolved dental source can act as a reservoir for ongoing infection, which may subsequently spread through the bloodstream or adjacent anatomical structures, putting vital organs at risk and complicating the clinical picture8. Furthermore, ignoring the dental origin of sinusitis not only increases the likelihood of recurrent or chronic sinus infections but also heightens the risk of severe complications, such as osteomyelitis or intracranial extension, due to the unchecked transfer of pathogenic bacteria from oral to sinus regions and beyond8. These interconnections underscore the necessity for clinicians to maintain a high degree of suspicion for dental causes in unilateral sinusitis presentations and to implement multidisciplinary diagnostic and therapeutic strategies to prevent serious health outcomes.

Treatment Approaches for Sinusitis of Dental Origin:

How should dental sources of infection be managed to resolve sinusitis?

Given the diagnostic challenges posed by the overlapping symptoms of dental-origin and typical sinusitis, a comprehensive approach to managing dental sources of infection is essential for effective resolution of sinusitis. Prompt identification and treatment of the underlying dental problem are crucial, as persistent or recurrent sinus infections may be driven by undetected dental infections such as a deeply decayed or abscessed tooth9,10. Addressing these dental sources typically necessitates professional dental intervention, which may include procedures like root canal therapy, extraction of the infected tooth, or surgical drainage if conventional treatments prove inadequate or if the infection becomes chronic9. The close anatomical relationship between the upper teeth and the maxillary sinuses means that oral bacteria can readily invade the sinus cavity, perpetuating sinus inflammation and infection unless the dental focus is properly managed9. Therefore, interdisciplinary collaboration between dentists and medical practitioners is recommended to ensure accurate diagnosis and timely treatment, ultimately preventing the recurrence of sinusitis and safeguarding both oral and sinus health. Early intervention and adherence to prescribed dental treatments are paramount to achieving full recovery and minimizing complications associated with dental-related sinusitis9,10.

What is the role of interdisciplinary collaboration in treating dental-origin sinusitis?

Given the unique etiology and presentation of dental-origin sinusitis, interdisciplinary collaboration is central to effective diagnosis and management. Odontogenic sinusitis, often stemming from dental infections such as failing root canals, frequently goes unrecognized unless dental and ENT specialists actively communicate and coordinate care11. This collaboration is crucial not only for accurate diagnosis—enabling comprehensive assessment through dental history, targeted physical examination, and nasal endoscopy—but also for developing tailored treatment plans that address both the dental source and the resultant sinus pathology11,12. Such a combined approach allows for simultaneous treatment of the underlying dental infection and associated sinus inflammation, which, in turn, reduces the need for multiple interventions and expedites recovery, ultimately minimizing patient morbidity and healthcare resource utilization11. Furthermore, joint surgical management by ENT and dental surgeons can address complex cases in a single operative session, preventing the recurrence of infection and reducing the risk of complications that might arise from isolated or sequential therapies11. The interdisciplinary model not only optimizes patient outcomes but also underscores the necessity for ongoing, structured cooperation between dental and otolaryngology teams, especially as the clinical complexity of odontogenic sinusitis often transcends the boundaries of any single specialty11,13. To ensure best practices, healthcare systems should prioritize protocols that facilitate seamless referral, communication, and shared decision-making between ENT surgeons and dental professionals, thereby enhancing both diagnostic accuracy and therapeutic success in cases of dental-origin sinusitis12,13.

 

DISCUSSION 

The findings of this comprehensive review underscore the critical importance of accurately diagnosing and managing sinusitis of dental origin to prevent potential complications and ensure effective patient outcomes. The overlap of clinical features between odontogenic sinusitis and other forms of sinus infections presents a significant diagnostic challenge, necessitating a high index of suspicion and thorough evaluation. The emphasis on specific dental indicators—such as localized pain associated with restorations or caries, along with radiographic evidence like periapical radiolucency—serves as vital clues that can differentiate dental sinusitis from other etiologies. Advanced imaging modalities, particularly cone-beam CT and AI-assisted radiography, have demonstrated considerable utility in enhancing diagnostic precision by delineating the proximity of dental lesions to the sinus floor and identifying subtle abnormalities that may be missed on conventional radiographs. The interdependence of dental and ENT specialties highlighted in this review reflects an essential interdisciplinary approach, which not only facilitates accurate diagnosis but also optimizes treatment planning. The management strategies discussed—ranging from dental interventions such as root canals and extractions to medical therapy and surgical procedures—reveal that a tailored, patient-centric approach yields the best outcomes. However, the review also exposes certain limitations, including potential biases in radiographic interpretation and the need for standardized diagnostic protocols across practices. Future research should focus on developing more precise, minimally invasive diagnostic tools, as well as establishing evidence-based guidelines for the interdisciplinary management of odontogenic sinusitis. Moreover, longitudinal studies are warranted to evaluate the long-term outcomes of combined dental and surgical treatments. Ultimately, fostering closer collaboration between dental and ENT specialists and increasing clinician awareness about the distinctive features of dental sinusitis will be vital in reducing misdiagnoses, preventing recurrent infections, and minimizing healthcare costs. This review contributes valuable insights into the nuanced diagnosis and management of odontogenic sinusitis, but continued research and clinical refinement are essential to fully address existing gaps and improve patient care standards.

 

CONCLUSIONS

This review highlights that sinusitis of dental origin is a distinct clinical entity often underdiagnosed due to overlapping features with rhinogenic sinusitis. Careful evaluation of dental history, clinical symptoms, and imaging studies such as cone-beam computed tomography (CBCT) are essential for accurate diagnosis and differentiation. Odontogenic sinusitis is most commonly associated with periapical infections, periodontal disease, and iatrogenic causes such as dental extractions or implant procedures. Treatment requires a dual approach, addressing both the underlying dental pathology and the sinus infection, which may involve endodontic therapy, dental extraction, or functional endoscopic sinus surgery (FESS) in refractory cases. Antibiotics alone are rarely curative without definitive management of the dental source. Overall, successful outcomes depend on interdisciplinary collaboration between dentists, oral surgeons, and otolaryngologists.

 

FUTURE DIRECTIONS

Future research should prioritize prospective, multicenter studies that directly compare the outcomes of different diagnostic imaging modalities and combined treatment approaches. There is a need to establish standardized diagnostic criteria to clearly differentiate odontogenic from rhinogenic sinusitis, reducing the risk of misdiagnosis and inappropriate treatment. Advances in minimally invasive surgical techniques, as well as biomarker research for early identification of odontogenic sinusitis, represent promising areas of exploration. Furthermore, long-term outcome studies assessing recurrence rates, patient-reported quality of life, and cost-effectiveness of various treatment strategies will be critical in guiding evidence-based practice. Ultimately, strengthening interdisciplinary collaboration and integrating precision medicine approaches will be essential in optimizing care for patients with sinusitis of dental origin.
