INTRODUCTION
Dermatological surgery has evolved significantly over recent decades, driven by technological advancements, a deeper understanding of skin pathology, and a growing demand for minimally invasive yet highly effective treatment options. Traditionally, surgical interventions such as excisions, cryotherapy, and electrosurgery have served as mainstays for treating benign and malignant skin conditions, providing reliable outcomes but often accompanied by limitations such as scarring and extended recovery times. In response, the field has witnessed a paradigm shift towards minimally invasive procedures, including laser therapies, micrographic surgeries, and novel energy-based devices, which aim to reduce patient discomfort, improve aesthetic results, and shorten healing periods. Evaluating the outcomes of these procedures involves not only assessing clinical effectiveness and complication rates but also integrating patient-reported outcomes that reflect satisfaction and quality of life post-treatment. Concurrently, innovations in dermatological surgery are propelled by cutting-edge technological developments—such as advanced laser systems, biocompatible materials, and digital imaging techniques—that enhance precision, safety, and postoperative recovery. These advancements are shaping future trends, promising more personalized and efficient surgical interventions.
OBJECTIVE
To explore the main surgical techniques used in dermatology, compare traditional approaches with innovative ones, evaluate the efficacy and complications associated with these procedures, and examine how emerging technologies and materials are transforming the landscape of dermatological surgery, with the ultimate goal of informing best practices and guiding future research in this dynamic field.
METHODOLOGY
This review is conducted by systematically analyzing the scientific literature on dermatological surgery, focusing on surgical techniques, clinical outcomes, and recent innovations. Peer-reviewed articles are identified through searches in PubMed, Scopus, and Web of Science, covering the period between 2000 and 2025. Keywords used include “dermatological surgery”, “surgical techniques”, “clinical outcomes”, “innovations” and “skin surgery.” Inclusion criteria consist of clinical trials, cohort studies, systematic reviews, and meta-analyses that assess procedural efficacy, safety, patient outcomes, and technological advancements. Exclusion criteria include case reports, pediatric-specific studies, and papers not directly addressing surgical outcomes or techniques. Data are extracted and reviewed independently by multiple authors to ensure reliability, with attention given to traditional and emerging techniques such as Mohs micrographic surgery, laser-assisted procedures, reconstructive methods, and minimally invasive innovations. The evidence is synthesized narratively to provide an overview of current practices and future directions in dermatological surgery.

The choice of surgical technique is intricately linked to the disease’s type, stage, and anatomical location, with procedures ranging from simple excisions and curettage to more advanced methods like Mohs micrographic surgery, which is prized for its tissue-sparing and high cure rate properties1. In the case of melanoma, surgical excision with appropriate margins remains the cornerstone of treatment, underscoring the central role of surgical intervention in achieving optimal patient outcomes1. Beyond oncologic applications, dermatologic surgery employs reconstructive and cosmetic methods—such as skin grafts, flaps, and laser resurfacing—not only to restore function but also to enhance the cosmetic appearance of affected areas, highlighting the specialty’s dual focus on both curative and aesthetic goals2. The interconnected nature of these domains requires dermatologic surgeons to balance disease eradication with the preservation of healthy tissue and the patient’s overall quality of life. As skin disorders frequently extend their impact to the hair and nails, surgical dermatology must also tailor its approaches to address these structures, emphasizing the need for ongoing advances, multidisciplinary collaboration, and patient education to ensure comprehensive care3.

The transition from manual laparoscopic methods to robotic platforms marks a pivotal shift, as robotic technology enhances visualization, manipulation, and ergonomics, thereby facilitating more complex and delicate interventions with improved patient outcomes4,5. This shift is not limited to a single domain but extends to multiple fields, including gynecologic, colorectal, pediatric, and spine surgery, where the adoption of minimally invasive methods has led to reduced tissue damage, minimized recovery times, and fewer complications compared to traditional open approaches4. Moreover, the development of specialized instruments—such as wristed devices and vessel sealing tools—has further refined the precision and versatility of these procedures, enabling surgeons to perform intricate tasks through small incisions8. Yet, the flexibility and rapid evolution afforded by the lack of strict regulation in surgical innovation can also pose challenges, as ongoing development and variable adoption rates may impede consistent evaluation of efficacy and safety6. Furthermore, the absence of a formal, sequential regulatory framework complicates the timing of validation and long-term monitoring, potentially impacting patient safety and the standardization of new techniques6. Thus, while traditional approaches remain indispensable for certain complex procedures and resource-limited environments, the integration of robust yet adaptable evaluation systems for innovative techniques is essential to balance progress, safety, and accessibility across the surgical landscape6.

Importantly, for surgeries such as Mohs, the evaluation of tissue around the surgical margins is a critical metric, as it not only confirms complete excision of cancerous cells but also minimizes unnecessary removal of healthy tissue, thereby reducing morbidity and recurrence rates9. These interconnected domains—clinical efficacy, cosmetic outcome, and surgical proficiency—collectively inform the ongoing evolution of assessment protocols. Therefore, ongoing education, investment in new technologies, and fostering collaborative practice models will be essential actions to optimize both clinical outcomes and patient experiences in the future landscape of dermatological surgery.
DISCUSSION
The comprehensive analysis presented in this study underscores the pivotal role of surgical techniques in the effective management of dermatological conditions, particularly skin cancers such as melanoma and non-melanoma variants. The findings reaffirm that traditional excisional surgery remains a cornerstone of treatment due to its reliability, ease of use, and accessibility, especially in resource-constrained settings. However, the emergence of advanced modalities like Mohs micrographic surgery exemplifies how tissue-sparing approaches can optimize oncologic control while minimizing cosmetic and functional deficits. The integration of reconstructive and cosmetic procedures further highlights the importance of balancing disease eradication with aesthetic and quality-of-life considerations. Notably, technological innovations, including robotic-assisted systems, AI diagnostics, and novel materials, have demonstrated significant potential to enhance surgical precision, reduce trauma, and shorten recovery times. These advancements are particularly promising in expanding access to high-quality care through telemedicine and minimally invasive techniques, which are increasingly vital given the growing demand for personalized and patient-centered treatment. Despite these technological strides, the study reveals that traditional surgical methods retain their relevance, especially in settings with limited resources, though they are associated with larger scars and longer recuperation. The challenges faced by newer techniques—such as regulatory hurdles, high costs, and the need for rigorous validation—must be addressed to facilitate broader adoption. Importantly, the emphasis on standardized metrics for evaluating surgical outcomes, including patient-reported measures, enhances our understanding of treatment effectiveness from the patient’s perspective. Nonetheless, the study’s limitations include variability in outcome reporting and a relative paucity of long-term data on the durability of emerging technologies. Future research should focus on large-scale, multicenter trials to validate the efficacy and safety of innovative approaches and explore how personalized medicine and 3D printing can further refine surgical accuracy. Additionally, fostering multidisciplinary collaboration and continuous professional development is essential for integrating these advancements into routine practice. Overall, while technological progress offers exciting opportunities for improving dermatological surgery, ensuring equitable access, safety, and efficacy remains paramount. Addressing these challenges will be crucial for translating innovations into tangible benefits for diverse patient populations and advancing the field toward more precise, minimally invasive, and patient-centered care.
CONCLUSIONS
Dermatological surgery has undergone significant evolution, integrating traditional procedures with advanced technologies to enhance precision, safety, and patient satisfaction. Techniques such as Mohs micrographic surgery continue to demonstrate exceptional efficacy in skin cancer management, while minimally invasive approaches and laser-assisted modalities offer improved cosmetic outcomes and faster recovery. Reconstructive procedures play a critical role in restoring both form and function, particularly in complex cases involving large excisions or cosmetically sensitive areas. Overall, the outcomes of dermatological surgery are highly favorable when techniques are tailored to individual patient needs, supported by advances in imaging, instrumentation, and perioperative care. Nevertheless, variability in patient-reported outcomes and the persistence of complications such as scarring, infection, and recurrence highlight the ongoing need for refinement.
FUTURE DIRECTIONS
