Este artículo tiene como objetivo explorar las mejores prácticas en la preparación para la colonoscopia y las últimas innovaciones en la atención de seguimiento, destacando en última instancia cómo estos factores convergen para mejorar los resultados y la experiencia del paciente en los procedimientos de colonoscopia.

INTRODUCTION
Colonoscopy procedures are a critical component of modern gastrointestinal healthcare, recognized for their role in early detection and prevention of colorectal cancer, as well as in the diagnosis of various gastrointestinal disorders. However, the effectiveness of these procedures is heavily influenced by the preparation and follow-up care received by patients. Inadequate preparation can lead to insufficient visualization of the colon, resulting in missed lesions and the potential for misdiagnosis, while effective follow-up care is essential for addressing complications, ensuring appropriate recovery, and enhancing overall patient satisfaction. Key steps in the preparation phase include dietary restrictions and bowel cleansing protocols, which are paramount for achieving optimal results. Recent innovations, such as advancements in bowel preparation agents and digital health tools, have shown promise in improving patient compliance and adherence to these protocols, thereby enhancing the quality of the procedure. Furthermore, the importance of follow-up care cannot be overstated; it serves as a bridge between the procedure and the patient’s ongoing health management, where personalized approaches and technological advancements, including telehealth and mobile health applications, can significantly improve patient outcomes and experiences.
OBJECTIVE
Explore best practices in colonoscopy preparation and the latest innovations in follow-up care, ultimately highlighting how these factors converge to improve outcomes and patient experiences of colonoscopy procedures.
METHODOLOGY
This article was designed as a narrative literature review aimed at synthesizing current evidence on best practices and recent innovations to enhance patient outcomes and experiences in colonoscopy preparation and follow-up care. A structured search was conducted in PubMed/MEDLINE, Embase, Web of Science, Cochrane Library, and Google Scholar for English-language articles published between January 2005 and November 2025. Search terms and Boolean combinations included “colonoscopy”, “bowel preparation”, “split-dose regimen”, “low-volume bowel prep”, “patient experience”, “patient satisfaction”, “patient-reported outcomes”, “follow-up care”, “surveillance colonoscopy”, “quality indicators”, and “digital health interventions”.
Original research articles (randomised controlled trials, cohort and case–control studies), systematic reviews, meta-analyses, clinical practice guidelines, and high-quality narrative reviews focused on adult patients undergoing colonoscopy for screening, surveillance, or diagnostic purposes were considered eligible. Studies dealing primarily with paediatric populations, other endoscopic procedures, non–peer-reviewed materials, conference abstracts without full text, and articles not available in English were excluded.

What are the key steps involved in preparing a patient for a colonoscopy?
A critical element in the preparation for a colonoscopy is ensuring the colon is entirely clean, which is achieved through adequate bowel preparation1. This process usually involves taking strong laxatives or bowel irrigation solutions, which are designed to flush the colon completely2. It is essential for the bowel to be thoroughly cleansed to accurately detect any abnormalities, polyps, or signs of disease, which underscores the importance of this preparation step2. Patients are typically required to make dietary modifications, such as adopting a clear liquid diet the day before the procedure, to aid in the cleansing process1. They must also fast on the day of the procedure to ensure a clear view of the colon2. Proper preparation contributes significantly to the success of the colonoscopy, as skipping any part of the preparation can impact the results and may necessitate a repeat procedure2. Therefore, following the recommended preparation guidelines is paramount, not only to facilitate an effective and safe colonoscopy but also to ensure early detection and prevention of potential gastrointestinal issues2,3.
How do dietary restrictions impact the effectiveness of colonoscopy preparation?
Dietary restrictions play an integral role in the effectiveness of colonoscopy preparation, highlighting the need for comprehensive guidelines and patient adherence to optimize outcomes. The adherence to dietary restrictions, such as a low-residue diet, has been shown to significantly enhance the quality of bowel preparation, thereby ensuring a clear view of the colon during the procedure4. Despite the clear benefits, adherence to these dietary guidelines remains a significant challenge, with only 27.8% of patients in some studies following the prescribed instructions5. This non-compliance can lead to inadequate bowel preparation, which is a common issue, with nearly half of patients experiencing suboptimal results as indicated by the Aronchick scale5. The difficulty of adhering to dietary restrictions, such as avoiding high-fiber foods and certain vegetables, further complicates the preparation process and negatively impacts patients’ quality of life6. Furthermore, the unpleasantness of dietary restrictions can discourage patients from repeating the test, posing a risk to timely detection and prevention of colorectal cancer6. To address these challenges, a more structured approach to dietary intake is necessary, with clear communication and education on the importance of dietary adherence for effective colonoscopy preparation5.
What innovations have improved patient compliance with colonoscopy preparation protocols?
Innovations in colonoscopy preparation protocols have focused on improving patient compliance, addressing challenges like dietary restrictions that were highlighted in previous discussions. Smart Patient Applications (SPAs) have emerged as a significant development in this area, enhancing adherence by integrating features such as automatic alerts and reminders which help ensure that patients follow dietary instructions and laxative regimens more effectively7. These applications not only provide timely prompts but also offer ongoing reviews of bowel preparation instructions, which can mitigate the complexities faced by patients and improve the overall quality of the preparation process7. By leveraging technology, patients receive continuous education and guidance, which has been shown to lead to better procedure quality outcomes, thus addressing the gap in adherence noted earlier7. Moreover, the introduction of low-volume, split-dose regimens as part of the preparation protocol offers another layer of convenience and compliance, as patients often find these regimens easier to tolerate and adhere to8. Such innovations collectively empower patients, leading to improved compliance and subsequently more effective colonoscopy procedures, highlighting the need for continued integration of technology and patient-centric approaches in medical protocols.

Follow-up care post-colonoscopy is pivotal in enhancing patient outcomes by ensuring that potential issues are addressed promptly and effectively. The results of a follow-up colonoscopy are instrumental in guiding management strategies for individuals with a history of polyps, enabling healthcare providers to tailor interventions that specifically target the prevention of colorectal cancer9. Moreover, the surveillance interval may be adjusted based on the findings of the follow-up colonoscopy, ensuring that patients receive the most appropriate monitoring schedule, which is crucial for early detection and intervention9,10. This personalized approach is further strengthened by open communication between patients and healthcare providers, which is essential for patients to fully understand the significance of follow-up procedures and comply with recommended care plans9. Lifestyle modifications, suggested during follow-up care, also play a significant role in enhancing patient outcomes by promoting healthier living and reducing risk factors associated with colorectal conditions9. By prioritizing timely and effective follow-up care, healthcare providers can significantly mitigate the risk of colorectal cancer, ultimately improving patient safety and long-term health outcomes9.
What recent technological advancements have been integrated into follow-up care for colonoscopy?
With the completion of bowel preparation, recent technological advancements have transformed the follow-up care associated with colonoscopy, enhancing both the precision and comfort of these procedures. High-definition technology is a pivotal innovation that has been integrated into colonoscopy follow-ups, allowing for more detailed visualization of the colon’s lining, which is critical for identifying polyps or other abnormalities that may have been missed in lower-definition images11. This enhanced imaging capability not only improves diagnostic accuracy but also reduces the likelihood of needing repeat procedures, thus contributing to more effective patient management. Additionally, advancements in the maneuverability of the instruments used during colonoscopies have significantly improved the procedure’s safety and efficiency11. The improved design of these instruments allows for greater precision and control, minimizing discomfort for the patient and reducing procedure time. Together, these technological advancements underscore the importance of integrating cutting-edge tools into follow-up care, ensuring that colonoscopy remains a vital component of gastrointestinal health management. These innovations highlight the need for continuous investment in technology to further refine and improve the outcomes of colonoscopy procedures.
How does personalized follow-up care improve patient experiences after a colonoscopy?
Personalized follow-up care after a colonoscopy plays a crucial role in addressing the diverse needs of patients, thereby improving their overall experiences and outcomes. By developing a tailored follow-up plan, healthcare providers can directly address any concerns that may have arisen during the procedure, thereby improving the patient’s understanding and management of their health12. This customized approach not only fosters a proactive relationship between the patient and doctor but also ensures that patients receive specific attention based on their individual results and health conditions12. For instance, if any abnormalities are detected, personalized care can include scheduling repeat colonoscopies or additional imaging studies, thus aiding in the monitoring and prevention of potential complications12. In more serious cases, the follow-up care may involve recommendations for surgical interventions, ensuring that significant findings are promptly addressed to improve patient outcomes13. Moreover, for those with inflammatory conditions, personalized medication prescriptions can significantly enhance patient comfort and optimize recovery post-procedure12. In essence, personalized follow-up care ensures a comprehensive, patient-centered approach that not only addresses immediate concerns but also contributes to long-term health monitoring and improvement.
DISCUSSION
The findings of this research underscore the critical role of effective colonoscopy preparation and follow-up care in enhancing patient outcomes and experiences. The necessity for thorough bowel cleansing prior to colonoscopy cannot be overstated, as inadequate preparation can lead to missed diagnoses and the need for repeat procedures, which not only burdens healthcare resources but also increases patient anxiety and discomfort. Our study highlights the significant challenges associated with patient adherence to preparation protocols, particularly regarding dietary modifications and fasting requirements. Despite the introduction of Smart Patient Applications (SPAs) and low-volume, split-dose regimens designed to improve compliance, the persistence of non-adherence indicates a need for further investigation into the barriers patients face. It is essential to explore how these innovations can be optimized to cater to diverse patient populations, particularly those with varying levels of health literacy and support systems. Furthermore, the importance of tailored follow-up care for patients with a history of polyps emphasizes the necessity for personalized healthcare approaches that consider individual risk factors and lifestyle choices. Open communication between healthcare providers and patients is vital in ensuring that the significance of follow-up care is fully understood, promoting engagement in preventive measures against colorectal cancer. As technology continues to evolve, the advancements in imaging and instrument maneuverability present exciting opportunities to enhance diagnostic accuracy and patient comfort, yet they also necessitate ongoing training and adaptation by healthcare professionals. Acknowledging the limitations of our study, such as the variability in patient populations and institutional practices, we advocate for future research to focus on longitudinal studies that assess the long-term impact of innovative preparation and follow-up strategies on patient outcomes. Ultimately, this research contributes to the growing body of knowledge in gastrointestinal health management, reinforcing the necessity for continuous innovation and patient-centered care in the pursuit of improved colorectal health outcomes.
CONCLUSIONS
- High-quality colonoscopy today depends not only on technical execution but on the entire care pathway, from preparation to follow-up. Evidence consistently shows that adequate bowel preparation is a key determinant of mucosal visualisation, adenoma detection, and the need for repeat procedures, directly influencing colorectal cancer prevention. Consensus guidelines and recent updates emphasise split-dose regimens and, when appropriate, low-volume polyethylene glycol–based or alternative formulations, which can achieve equal or superior cleansing with better tolerability and higher patient satisfaction.
- From the patient’s perspective, discomfort, complexity of instructions, and fear of adverse effects remain major barriers to adequate preparation and screening uptake. Patient-centred strategies—such as simplified written and visual instructions, reminder systems (SMS, apps, phone calls), culturally and linguistically adapted education, and structured support for high-risk or hospitalized patients—have been associated with fewer inadequate preps, fewer repeated colonoscopies, and improved overall experience. These approaches align with a broader movement towards measuring and improving patient-reported outcomes alongside traditional technical quality indicators.
- Follow-up care after colonoscopy is equally crucial for long-term outcomes. Current guidelines advocate risk-stratified surveillance intervals based on baseline findings and quality of the initial examination, seeking to avoid both underuse and overuse of repeat colonoscopy. Quality indicators—such as adenoma detection rate, cecal intubation rate, standardized scoring of bowel prep quality, and appropriate documentation of recommended surveillance intervals—are now central to assessing performance and predicting interval cancer risk.
