INTRODUCTION
Hydrocele, a condition characterized by the accumulation of fluid in the tunica vaginalis surrounding the testis, represents a significant clinical concern for many patients, warranting comprehensive understanding of its underlying causes, symptomatic expressions, and therapeutic interventions. The etiology of hydrocele is multifaceted, encompassing both congenital factors—such as developmental abnormalities during fetal growth—and secondary causes that arise in adulthood, including trauma, infections, or tumours. This condition often manifests through observable symptoms like scrotal swelling and discomfort, which can severely impact the daily lives and activities of affected individuals, prompting the need for effective diagnostic methods to differentiate hydrocele from similar conditions. The exploration of treatment options is equally critical; while non-surgical alternatives such as aspiration and compression may offer temporary relief, the need for surgical intervention becomes apparent in cases where hydrocele is recurrent or symptomatic. Understanding the potential risks and benefits associated with various treatment modalities not only aids in guiding clinical decision-making but also enhances patient education and management strategies.
OBJECTIVE
To provide a holistic perspective that can inform healthcare professionals and improve patient outcomes in the management of this prevalent condition.
METHODOLOGY
This scientific review investigates the causes, symptoms, and treatment options for hydrocele, with the aim of providing a comprehensive understanding of this condition. A systematic literature search is conducted using major electronic databases, including PubMed, Scopus, and Web of Science, to identify peer-reviewed articles, clinical studies, systematic reviews, and meta-analyses published within the past two decades. Search terms such as “hydrocele etiology,” “hydrocele symptoms,” “management of hydrocele,” “surgical treatment for hydrocele,” and “non-surgical hydrocele treatment” are employed. Additional references from relevant articles and clinical guidelines are also reviewed to ensure a broad and inclusive analysis.
The review includes studies focusing on both primary and secondary hydroceles, addressing their underlying causes, such as congenital factors, trauma, infections, or systemic conditions. Clinical presentations, including asymptomatic cases, scrotal swelling, and discomfort, are systematically analyzed to highlight the variability in symptomatology. Diagnostic approaches, including physical examination, imaging techniques such as ultrasound, and laboratory investigations, are evaluated to identify the most effective methods for accurate and timely diagnosis.
Treatment options are a central focus of the review, encompassing conservative management, minimally invasive procedures, and surgical interventions. Studies on needle aspiration, sclerotherapy, and various surgical techniques, including hydrocelectomy, are examined for their efficacy, safety, and long-term outcomes. Comparative analyses of these methods provide insights into their applicability in different patient populations and clinical scenarios.
Inclusion criteria prioritize articles that report on patient outcomes, such as symptom resolution, recurrence rates, and complications. To further confirm the diagnosis, the provider might employ the technique of transillumination, where a light is shined through the scrotum; if the scrotal sac appears to be filled with fluid, it suggests the presence of a hydrocele9. In cases where the physical examination and patient history are inconclusive or if there are concerns about other underlying conditions such as epididymitis, testicular torsion, or tumours, additional diagnostic methods may be necessary1. These can include imaging studies like ultrasounds, which are particularly useful for visualizing the fluid-filled sac and excluding other potential medical or surgical conditions1,10. Studies have shown that this combination is particularly effective, offering a success rate of 84% with a single treatment11. Not only does this method provide outcomes comparable to hydrocelectomy, it also avoids the hospital expenses and complications associated with surgical intervention11. The use of doxycycline in sclerotherapy has further enhanced the efficacy of the procedure, marking a significant improvement over earlier methods that employed tetracycline, which had a success rate of 75%12. Therefore, there is a need for increased awareness and utilization of this procedure among healthcare providers to offer patients a viable alternative to surgery.

The persistence of the processus vaginalis, a congenital defect, plays a pivotal role in the formation of congenital hydroceles, highlighting the necessity for early diagnosis and intervention in affected newborns. The study emphasizes that while hydroceles are predominantly benign, their development can signal underlying pathological conditions such as infections, including epididymitis and orchitis, or even malignancies that necessitate vigilant evaluation. This research also draws attention to the often-overlooked non-surgical treatment alternatives, such as aspiration and sclerotherapy, which can provide effective symptomatic relief while minimizing the risks associated with operative procedures. Despite their potential benefits, these methods remain underutilized, indicating a need for increased awareness and education among healthcare providers regarding non-invasive treatment options. Future research should focus on the long-term outcomes of non-surgical interventions compared to traditional surgical approaches, as well as the development of guidelines for the management of hydroceles in diverse patient populations. By framing hydrocele within this broader context, the findings of this paper contribute significantly to the existing body of knowledge and underscore the importance of a comprehensive, individualized approach to treatment that prioritizes patient well-being and informed decision-making. Ultimately, this discussion serves as a springboard for further exploration into optimizing hydrocele management strategies and improving patient outcomes in clinical practice.

Early diagnosis, achieved through thorough clinical examination and imaging techniques like ultrasound, is essential for differentiating hydrocele from other scrotal pathologies and ensuring appropriate management.
