Este artículo de investigación tiene como objetivo proporcionar una comprensión más profunda de los desafíos que enfrentan tanto los pacientes como los proveedores de atención médica en el ámbito de la cirugía plástica, buscando en última instancia mejorar la atención al paciente y mejorar los resultados quirúrgicos.

INTRODUCTION

Plastic surgery has become increasingly prevalent, offering individuals the opportunity to enhance their physical appearance and address various aesthetic concerns. However, as with any surgical intervention, the risk of infections and dermatological complications poses significant challenges that can impact patient outcomes and satisfaction. Understanding the multifaceted risk factors associated with post-operative infections is crucial; these include patient-related factors such as age, comorbidities, and behavioral habits, as well as surgical techniques and the environments in which procedures are performed. Moreover, the spectrum of dermatological complications that can arise following plastic surgery, ranging from minor irritations to severe skin infections, necessitates a comprehensive examination of how specific surgical techniques contribute to these issues and how individual patient skin types may exacerbate or mitigate risks. Effective management strategies are paramount in addressing these complications, encompassing prevention measures to minimize infection rates and protocols for the treatment of dermatological issues. By systematically analyzing the interplay between risk factors, complications, and management strategies.

OBJECTIVE

To provide a deeper understanding of the challenges faced by both patients and healthcare professionals in the field of plastic surgery, with the ultimate goal of improving patient care and surgical outcomes.

METHODOLOGY

In conducting this scientific review on infections and dermatological complications following plastic surgery, a comprehensive and systematic methodology is employed to ensure the inclusion of relevant, high-quality literature. The review begins by identifying key research questions focused on the incidence, risk factors, clinical manifestations, and management strategies associated with postoperative infections and skin-related complications in plastic surgery patients. A thorough search is performed using major electronic databases, including PubMed, Scopus, Web of Science, and Google Scholar. The search strategy incorporates a combination of medical subject headings (MeSH) and keywords such as «plastic surgery», «postoperative infections», «dermatological complications», «wound healing», «risk factors» and «management strategies.»

Inclusion criteria consist of peer-reviewed articles published in English within the last 15 years, covering human subjects and focusing on both elective and reconstructive plastic surgical procedures. Exclusion criteria include case reports, editorials, and studies with limited methodological quality or lacking relevance to the primary objectives of this review. The selection process is carried out in two stages: an initial screening of titles and abstracts, followed by a full-text review of eligible studies. Data are extracted systematically and include study design, population characteristics, types of surgical procedures, types and incidence of complications, risk factors, treatment approaches, and clinical outcomes.

Findings are synthesized thematically, emphasizing the most frequently reported infections (e.g., cellulitis, abscesses, necrotizing fasciitis) and dermatological issues (e.g., delayed wound healing, scarring, skin necrosis), as well as prevention and treatment modalities. This methodology allows for a critical evaluation of existing literature and aims to provide healthcare professionals with a well-rounded understanding of the challenges and solutions in managing dermatological complications following plastic surgery.

Obesity not only complicates the surgical procedure itself but also affects the body’s healing capacity, thereby increasing susceptibility to infections. Furthermore, the pattern of infections can vary significantly depending on the surgical procedure and the operative approach taken3. For instance, more invasive procedures or those involving longer operative times may have higher infection rates due to greater tissue manipulation and exposure. Additionally, geographical location plays an integral role, as it can influence the prevalence and types of infectious agents patients are exposed to, thus affecting post-operative infection rates3. This variability underscores the importance of tailoring preoperative and postoperative care to address these regional differences effectively. Ultimately, categorizing the risk factors into patient and procedural factors allows for a more comprehensive assessment and management strategy, leading to improved prevention and treatment protocols for postoperative infections in plastic surgery3. Addressing these factors through careful patient evaluation and procedural planning is essential to minimize the risk and enhance surgical outcomes.

Additionally, surgical environments should implement proper ventilation systems to minimize airborne contamination, which is a known contributor to surgical site infections9. Surgical techniques also play a crucial role, where aseptic methods are necessary to reduce infection risks during operations10. This comprehensive approach, which integrates both advanced surgical techniques and meticulously maintained environments, is vital in significantly reducing the incidence of surgical site infections11. Ultimately, these efforts not only enhance patient safety but also contribute to more successful surgical outcomes by minimizing the infection risk and promoting better recovery10.

Dermatological Complications Post Plastic Surgery:

While many lesions that manifest post-surgery are transient and considered an inherent part of the recovery process, they necessitate proper evaluation to prevent escalation into more severe conditions13. The role of antiseptic solutions in preoperative preparation cannot be overstated, as ensuring these solutions are fully dry before commencing surgery significantly reduces the risk of adverse skin reactions13. Additionally, bleeding, a common complication in dermatologic procedures, can evolve into critical issues such as hematoma, infection, and skin necrosis, especially when compounded by mechanical factors like hypertension and trauma14. The potential for skin necrosis is particularly heightened in patients who smoke, have diabetes, or are overweight, necessitating careful preoperative assessment and postoperative monitoring to manage this risk effectively15. Overall, these complications underscore the importance of meticulous surgical technique, comprehensive patient evaluation, and diligent postoperative care in mitigating dermatological issues after plastic surgery.

The geographical differences in infectious agents also warrant consideration of localized protocols for preoperative and postoperative care, ensuring that strategies are relevant to the specific patient population. The role of surgical techniques and environmental factors cannot be overstated; our findings stress the necessity of maintaining rigorous sterile practices in the operating room to counteract the persistent threat of pathogens. While educational initiatives aimed at informing patients about potential complications are vital, further exploration into effective communication strategies and their impact on patient outcomes is necessary. The study also identifies gaps in the current management of dermatological complications, particularly regarding the use of prophylactic antibiotics, which challenges conventional practices and suggests a shift towards alternative wound care solutions. Limitations of this study include the potential biases in patient reporting and the variability in surgical techniques employed across different practitioners. Future research should focus on developing standardized protocols that encompass all identified risk factors and evaluate the effectiveness of interventions in real-world settings. A systematic and proactive approach to risk management, as advocated in this research, is essential not only for enhancing patient safety but also for fostering continuous improvement in the quality of care within the field of plastic surgery.

CONCLUSIONS

  1. Infections and dermatological complications remain significant concerns in plastic surgery, impacting patient outcomes and recovery. This comprehensive review highlights the multifactorial nature of these complications, with risk factors including patient-related factors (such as diabetes, smoking, and immunosuppression), surgical-related factors (such as wound tension, surgical technique, and implant use), and environmental factors (such as hospital hygiene and post-operative care). The incidence of infections, including cellulitis, abscesses, and more severe complications like necrotizing fasciitis, varies depending on the procedure and the patient’s risk profile. Additionally, dermatological issues such as delayed wound healing, scarring, and skin necrosis are common and can significantly affect aesthetic outcomes and patient satisfaction.
  2. Effective prevention and management strategies are critical in reducing these complications. Optimizing preoperative care, including proper patient selection, smoking cessation, and management of comorbidities, is essential. Intraoperative techniques, such as sterile practices, judicious use of drains, and careful handling of tissues, help minimize infection risk. Postoperative care, including appropriate antibiotic prophylaxis, vigilant monitoring for signs of infection, and prompt intervention when complications arise, plays a key role in improving outcomes. Furthermore, strategies like wound care protocols, use of advanced dressing materials, and techniques to optimize scar healing contribute to reducing dermatological complications.
  3. While much progress has been made in understanding the risk factors and developing management protocols, challenges remain in individualizing care for high-risk patients and further improving clinical outcomes. Future research should focus on identifying novel biomarkers for early detection of infections, exploring advanced wound-healing technologies, and refining surgical techniques to prevent dermatological complications. By adopting a comprehensive, multidisciplinary approach to patient management, plastic surgeons can continue to reduce the incidence of infections and enhance aesthetic and functional outcomes for their patients.
