ABSTRACT
Dysphagia, or swallowing disorders, is a complex condition that significantly impacts the quality of life for millions of individuals worldwide, often leading to malnutrition, dehydration, and an increased risk of aspiration pneumonia. This condition can arise from a variety of causes, including neurological disorders such as stroke or Parkinson’s disease, which disrupt the neural pathways responsible for swallowing; structural abnormalities such as tumors or strictures in the esophagus, which can physically obstruct the passage of food; and muscular disorders like myasthenia gravis or muscular dystrophy, which impair the strength and coordination required for effective swallowing. Understanding these underlying causes is crucial for developing appropriate management strategies tailored to individual patients. Effective management of dysphagia typically involves a multidisciplinary approach, incorporating dietary modifications to ensure safe and adequate nutrition, swallowing therapy techniques designed to enhance swallowing function and safety, and, in severe cases, surgical interventions that can alleviate obstructions or improve anatomical function. This research paper aims to provide a comprehensive overview of the myriad causes of dysphagia, explore the symptoms that accompany this disorder, and critically evaluate the various management strategies available, ultimately contributing to a better understanding of how to support individuals affected by this challenging condition.

La disfagia, o trastornos de la deglución, es una afección compleja que afecta significativamente la calidad de vida de millones de personas en todo el mundo y que a menudo conduce a desnutrición, deshidratación y un mayor riesgo de neumonía por aspiración. Esta afección puede surgir por diversas causas, incluidos trastornos neurológicos como el accidente cerebrovascular o la enfermedad de Parkinson, que alteran las vías neuronales responsables de la deglución; anomalías estructurales como tumores o estenosis en el esófago, que pueden obstruir físicamente el paso de los alimentos; y trastornos musculares como la miastenia gravis o la distrofia muscular, que afectan la fuerza y la coordinación necesarias para una deglución eficaz. Comprender estas causas subyacentes es crucial para desarrollar estrategias de tratamiento adecuadas y adaptadas a cada paciente. El tratamiento eficaz de la disfagia generalmente implica un enfoque multidisciplinario, que incorpora modificaciones dietéticas para garantizar una nutrición segura y adecuada, técnicas de terapia de deglución diseñadas para mejorar la función y la seguridad de la deglución y, en casos graves, intervenciones quirúrgicas que pueden aliviar las obstrucciones o mejorar la función anatómica. OBJECTIVE
To provide an overview of the multiple causes of dysphagia, explore the symptoms that accompany this disorder and critically evaluate the various treatment strategies available.
METHODOLOGY
This scientific review aims to explore and synthesize current knowledge on dysphagia, focusing on its causes, symptoms, and management strategies for swallowing disorders. A comprehensive literature search is conducted using databases such as PubMed, Scopus, and Web of Science. The search employs keywords including «dysphagia», «swallowing disorders», «oropharyngeal dysphagia», «esophageal dysphagia», «dysphagia management», and «swallowing rehabilitation.» The scope of the review includes articles published in the past 20 years to ensure relevance to modern diagnostic and treatment practices.
The review focuses on the clinical manifestations of dysphagia, covering symptoms such as difficulty swallowing, coughing or choking during meals, weight loss, and aspiration pneumonia. Management strategies for dysphagia, ranging from swallowing therapy and dietary modifications to more invasive treatments like surgical interventions and the use of feeding tubes, are evaluated. The impact of multidisciplinary approaches, including the roles of speech and language therapists, dietitians, and gastroenterologists, is also considered in the review.
Studies are critically appraised for their methodological rigor, sample size, and relevance to clinical practice. Findings are organized into thematic sections, including the causes of dysphagia, symptomatology, and evidence-based management strategies. Particular attention is given to innovative therapies, such as electrical stimulation and neurorehabilitation techniques, as well as the challenges of managing dysphagia in aging populations and patients with complex comorbidities.
This review synthesizes findings to provide a comprehensive overview of dysphagia, identifying gaps in current research and outlining potential areas for future studies. By evaluating existing evidence on causes, symptoms, and management strategies, the review aims to inform clinicians about effective approaches to diagnosing and treating swallowing disorders.

What are the neurological causes of dysphagia?
Neurological causes of dysphagia encompass a wide array of conditions, each impacting different components of the nervous system and presenting unique challenges in clinical management. For instance, neurodegenerative diseases such as Parkinson’s disease and amyotrophic lateral sclerosis (ALS) are significant contributors to dysphagia, with about 50% of Parkinson’s patients and more than 30% of ALS patients experiencing swallowing difficulties in the early stages of their diseases1,2. Similarly, motor neuron diseases like bulbar and pseudobulbar palsy lead to progressively worsening dysphagia with minimal limb involvement, highlighting the complex interplay between motor function and swallowing mechanisms1,3. Central nervous system (CNS) disorders, including strokes and traumatic brain injuries, are also predominant causes, with at least 50% of stroke patients and 60% of those with traumatic brain injuries developing clinically relevant dysphagia1,4. These conditions often result in varied clinical presentations, such as oral and pharyngeal phase disturbances, depending on the location and extent of neurological damage2,5. The development of dysphagia is intricately linked to various muscle disorders, particularly those affecting the skeletal muscles involved in the swallowing process. The weakening of these muscles, which can occur due to conditions such as neuromuscular disease, stroke, and head and neck carcinoma, disrupts the complex coordination required for effective swallowing8. As individuals age, a notable decline in the mass of swallowing muscles, especially the genio-hyoid muscle and tongue, becomes apparent, further exacerbating the risk of dysphagia8. This condition, known as sarcopenic dysphagia, is characterized by the generalized loss of skeletal muscle mass, which substantially impairs swallowing function8. The prevalence of dysphagia among the elderly underscores the significant role muscle disorders play in its development, emphasizing the need for comprehensive management strategies that address muscle health, such as rehabilitation and nutritional support8. Thus, understanding and intervening in muscle disorders is crucial for mitigating the impact of dysphagia, particularly in vulnerable populations.
Management Strategies for Dysphagia:
What are the dietary modifications recommended for patients with dysphagia?
The dietary modifications recommended for patients with dysphagia are multifaceted and tailored to individual needs to ensure safe and efficient swallowing. Dysphagia diets are structured to address the varying degrees of swallowing difficulties experienced by patients and often involve altering both the texture of solid foods and the consistency of liquids9. For example, the National Dysphagia Diet Task Force has established a categorization system for modified liquids, dividing them into four levels: thin, nectar-like, honey-like, and spoon-thick10. Additionally, solid foods are often modified to be soft, moist, and easy to swallow, such as in a Dysphagia Soft or Minced diet9. These texture modifications are crucial for preventing aspiration and ensuring that patients can consume their meals comfortably. Skilled nursing facilities and hospitals typically implement these dietary modifications, with many providing dysphagia diets at multiple levels to cater to the specific needs of their patients10. Moreover, continuous monitoring and regular reassessment of a patient’s swallowing abilities are essential, as these abilities may change over time, necessitating adjustments to their diet10. How effective are swallowing therapy techniques in managing dysphagia?

These techniques can be broadly categorized into compensatory strategies and rehabilitative exercises. Compensatory strategies, such as the chin tuck, head tilt, and head turn, are designed to prevent the symptoms of dysphagia by modifying the swallowing process without inducing long-term physiological changes11. Although these strategies are effective in providing immediate relief, their benefits are not sustained over the long term11. In contrast, rehabilitative exercises, including the Shaker exercise and Masako maneuver, focus on enhancing the physiology of swallowing by increasing muscle strength, speed, and coordination11. These exercises follow principles of neuromuscular rehabilitation, such as repetition and muscle overload, to promote neuroplasticity and achieve lasting improvements12. Additionally, exercises like the effortful swallow and supraglottic swallow serve both compensatory and rehabilitative purposes, bridging the gap between immediate symptom relief and long-term physiological enhancement11. Among the various surgical options, laryngeal elevation, cricopharyngeal myotomy, and vocal fold medialization are frequently performed to address different underlying causes of dysphagia13. Laryngeal elevation is particularly effective in enhancing the swallowing mechanism by repositioning the larynx to facilitate a safer and more efficient swallowing process13. Meanwhile, cricopharyngeal myotomy involves the dilation of the contracted upper esophageal sphincter (UES), thereby improving the passage of food through the esophagus13. This procedure can be especially beneficial for patients with structural cricopharyngeal disorders, highlighting the importance of individualized treatment plans based on the specific etiologies of dysphagia14. Vocal fold medialization, another critical intervention, strengthens glottal closure during swallowing and phonation, which is vital for patients with unilateral vocal fold paralysis [13]. Despite the potential benefits, the outcomes of these surgical interventions have been mixed, with some procedures yielding less promising results than others14. Therefore, it is essential to carefully evaluate the severity of dysphagia and consider surgical options only after conservative treatments have failed14. This comprehensive approach allows for the optimal management of dysphagia, ensuring that patients receive tailored and effective treatment strategies to improve their quality of life.
DISCUSSION
The findings of this research highlight the multifactorial nature of dysphagia, particularly its strong association with various neurological and structural disorders. The significant prevalence of dysphagia among stroke and traumatic brain injury patients underscores the urgent need for targeted interventions within this population, as over half of these individuals experience clinically relevant swallowing difficulties. This data correlates with previous literature that recognizes neurodegenerative diseases, such as Parkinson’s disease and ALS, as critical contributors to dysphagia, indicating that early identification and management strategies are paramount in improving patient outcomes. The nuanced interplay between neurological impairment and muscle function, particularly in conditions like bulbar and pseudobulbar palsy, necessitates a more refined understanding of how these disorders progress and affect swallowing mechanics. Future research should focus on elucidating the underlying mechanisms of dysphagia across diverse populations and conditions, exploring innovative therapeutic approaches, and establishing standardized protocols that could lead to more uniform treatment outcomes in clinical practice. Overall, this discussion reinforces the importance of understanding dysphagia as a complex disorder that requires a multidisciplinary approach, combining medical, surgical, and rehabilitative strategies to optimize patient care and quality of life.

It significantly impacts quality of life and poses serious health risks, such as malnutrition, dehydration, and aspiration pneumonia, particularly in vulnerable populations like the elderly or individuals with neurological impairments. Understanding the underlying causes of dysphagia is critical for effective diagnosis and treatment, as the disorder can manifest in both the oropharyngeal and esophageal phases of swallowing, each requiring different management approaches.
- Accurate diagnosis is essential for identifying the specific type and cause of dysphagia, with tools such as videofluoroscopic swallow studies, fiberoptic endoscopic evaluation of swallowing (FEES), and esophageal manometry playing a crucial role. These diagnostic methods enable healthcare professionals to assess the mechanics of swallowing, detect abnormalities, and tailor treatment strategies accordingly. However, access to advanced diagnostic techniques may be limited in certain settings, highlighting the need for broader availability of these resources to improve patient outcomes.

Non-invasive treatments, such as swallowing therapy and dietary modifications, are effective for many patients, especially when integrated into a multidisciplinary approach involving speech and language therapists, dietitians, and physicians. As the global population ages, there is an increasing need for more research into effective dysphagia treatments that can be applied across diverse healthcare settings.
